Please complete the form below once for each asset (or share of an asset) that you want to put in to trust.
You can used this form at any time (not just when you are creating the trust) as you have an accumulation trust and the trustee has the power to accept new property in to the trust at any time.
Once you click submit, a settlement receipt will be emailed to you directly. Keep this settlement receipt with your trust as evidence of the asset being held on trust.
Also, don’t forget to update your asset listing spreadsheet.
